Why am I doing the OQ?

In an effort to maintain quality and effective care for you, we use Outcomes Measures Questionnaires to track your progress. 

 

When do I fill out the OQ?

We ask you to fill out a questionnaire prior to each session. Accessing your questionnaire is simple!

How do I fill out the OQ?

There will be 2 boxes for you to fill out:

Box #1: Identification number

Box #2: Your birthday

What is my identification number?

Your identification number is your first and last initial both capitalized (AA) and your birthday (MM/DD/YY)

 

What format is my birthdate in?

It will be in the format MM/DD/YYYY   

 

What is an example?                    

Ex. for Jane Doe born on April 1st, 2009

Jane's ID number is: JD040109

Jane's birth date is: 04/01/2009

Please ensure the correct OQ instrument is in the selected box

If you are 18 years or old, select the OQ- 45.2 (Outcomes Measures 45.2)

If you are the parent of a child under 18, login under your child's ID number and select the YOQ 30.2PR (Youth Questionnaire 30.2 Parental Report.

If you are under 18 years old select the YOQ 30.2SR (Youth Outcome Measures 30.2 Self Report)

To change the OQ Instrument, check the box before the instrument in the right. Move it back to the "available" section by clicking the "<" button between the two boxes. Next, check the box before the instrument you need from the "available" section click on the ">" button to move it to the "selected" section.
